The type of personal information collected
Thia Conn will be collecting personal information usually before or during your initial one to one or group session, (this refers to both face to face sessions & sessions via an online medium over the internet) Information will be collected via the online contact forms submitted at www.thiaconn.co.uk, via email correspondence, via the completion of your Intake & Release Form, over the phone (landline & mobile) & face to face. This is to ensure compliance with insurance requirements & also to make sure your health & safety is protected. Information gathered about you & your health is used to make sure sessions are suited to your individual requirements.
All information that is shared will be treated as confidential & in confidence & stored in compliance with General Data Protection Regulation (GDPR). Thia Conn will only retain information provided for as long as is necessary to fulfill the purposes it was collected for. This can also include legal, accounting, reporting & insurance requirements.
If you choose to not share information that is required it may prevent you from attending one to one sessions, classes, group classes or workshops.
Your Contact Information
Thia Conn currently collects & processes the following information; name, address, phone number, email address, relevant medical information & relevant medical history. Your details may be used to inform you of any unforeseen class time changes or cancellations & essential class or session details.
For face to face & online sessions & classes (one to one & group) photos & videos may be requested ahead of time for session planning purposes. Videos & photos will be treated as confidential & in confidence & stored in compliance with General Data Protection Regulation (GDPR). Thia Conn will only retain information provided for as long as is necessary to fulfill the purposes it was collected for.
When a mailing list is set up, email addresses may be used to alert you of new classes or offerings. This mailing list will be managed securely & you will be able opt out / unsubscribe at any time. By entering in your name & email address to subscribe to receive monthly tips & life hacks, latest online & in person offerings & exclusive discounts, you are agreeing to be emailed once per month to receive news & offers. You can opt out / unsubscribe anytime.
Some of your details may be stored on spreadsheets to keep track of session attendance (dates of sessions, length of sessions, costings) & progress notes. This is for insurance, accounting purposes & ease of reference while we are working together.
Emergency Contact Information
This information is only used in the case of an emergency. Thia Conn will use this to contact a next of kin, or a medical professional if required. Information will be passed over to medical professionals if it is required. Passing information to a third party would only be done in these circumstances & only in an emergency situation.
Data Security
Any information or personal data I collect is stored in compliance with General Data Protection Regulation (GDPR). Spreadsheets & files are backed up, password protected & stored securely. Submitted forms, photographs, videos will be stored on a password protected external hard drive.
They will be stored no longer than is required for the purposes they were collected for & no longer than GDPR permits. At which time information will be permanently deleted.
